WAIVER FORMS

Youth and adult player waiver forms are needed for every player.  You can download the 'PDF" version of these forms below, print them out and have each player fill them out.  They are required for participation in this event.

These forms require Adobe Reader software to read.  Most computers are equipped with this software, if your's does not have it, it is very easy to download right from here for free.
